Data Collection and Broadcast in Clinical Reasoning Using VGA Broadcaster Lite

Clinical reasoning is a dynamic process that occurs before, during, and after the collection of data through physical examination, imaging, endoscopic and laboratory tests. It is a major component of clinical competence. Studying this process is essential for better communicating the work to clients and colleagues, revealing to students the nuances of therapy that cannot be gained from texts and extending the knowledge of occupational therapy practice. A novel method of data collection for clinical reasoning involving a head-mounted video camera used in a large medical centre is described in this article. This method is very efficient in cases when along with data collection it is preferable to get a consultation of a remote specialist using an effect of telepresence. Participation of this specialist in the examination of a patient and debriefing is ensured by telecommunication equipment from Epiphan.

Concurrent reporting can interfere with the conduct of a therapy session. Therefore techniques that require the therapist to recall his or her reasoning after a session are commonly used. Video-assisted recall requires the subject to talk about what he or she was thinking as well as about what happened during the examination. There are two procedures: data capture and broadcast using video and VGA Broadcaster Lite as well as video-assisted recall. During recall the therapist watches the footage to facilitate recall of his or her thoughts, choices, and during debriefing discusses details with an expert who remotely participated in the examination. 

When capturing a therapy session on video, researchers are likely to use a stand-alone video camera although this approach to video data collection is limited in several ways. For example, the camera operator needs to be present to keep the subjects in centre view. Besides, the camera’s perspective is different from that of the therapist. These limitations are overcome with a new approach to data capture using a head-mounted video camera. The microphone records all sound. The therapist and patient are completely mobile. VGA Broadcaster Lite captures and broadcasts both video and audio over Ethernet. The remote expert having access to the broadcast sees everything that occurs during the therapy session.

An approach that uses data captured from a head-mounted camera has many advantages. When viewing footage from a head-mounted video camera, the therapist “sees” exactly what he or she saw as therapy unfolded. Own-point-of-view recall provides an accurate record of the therapist’s perspective of the client and the activity. The video feed is broadcast by VGA Broadcaster Lite together with a superimposed therapist’s narration of the session. Using a head-mounted camera together with a broadcasting appliance (such as VGA Broadcaster Lite) is an excellent method of eliciting reasoning. It ensures an external expert or diagnostician to have a clear picture of the session from the therapist’s own point of view and take part in the examination and debriefing.
